WASHINGTON (Reuters) - The first major study to evaluate the long-term safety of doxycycline in patients with rheumatoid arthritis has been halted after a new study showed it may have no negative effect on the risk of tendon ruptures.
The results of the first large, long-term study showed it had no negative effect on the risk of tendon ruptures. The study, published in March, compared doxycycline with aspirin, which can cause a drop in blood pressure, to aspirin alone, which can cause a drop in blood pressure. The new results are presented today at the American College of Rheumatology annual meeting.
Dr. Robert L. Molloy, a Professor of Medicine and a clinical statistician at Northwestern University, and colleagues at the University of Illinois at Chicago and the University of Chicago Medical Center analyzed data from two placebo-controlled trials for patients with rheumatoid arthritis who did not receive doxycycline, including those treated with aspirin. In the first trial, patients with a history of tendon problems were treated with doxycycline at 1 mg/kg/day and aspirin at 2.5 mg/kg/day. In the second trial, patients who had never taken aspirin and who did not have a history of tendon problems were treated with aspirin and doxycycline. No difference between the two groups was observed.
In the first placebo-controlled trial, those who received doxycycline were less likely to develop a tendon rupture than those who received aspirin. The results were analyzed in three separate, double-blind, multicenter trials that were conducted in the United States, Japan and Germany. The investigators were able to isolate these differences in the three studies by analyzing the risk of tendon ruptures in the two groups, with the exception of the first trial, which was done in Japan.
The authors said the results do not prove that doxycycline is associated with an increased risk of tendon rupture in the first trial, as the risk was greater for those who had never taken aspirin. The researchers said their results also should not be considered as definitive, as the risk was higher for those who had never taken aspirin.
"While there is insufficient evidence to support the use of doxycycline for the prevention of tendon injury, the findings of this new study suggest that it may be safe to use," said David A. Hirsch, MD, professor of rheumatology and a senior author on the study. "Because doxycycline is the most widely used drug in the United States for the prevention of rheumatoid arthritis, its use for the treatment of rheumatoid arthritis should be carefully evaluated."
Hirsch said the new results provide some new information to support the use of doxycycline in rheumatoid arthritis patients for long-term protection against tendon rupture. "Doxycycline is the drug of choice for rheumatoid arthritis because of its high safety profile and long half-life," Dr. Molloy said. "However, it may not be appropriate for patients with chronic conditions where other drugs have been associated with a low risk of tendon rupture."
The new study is the first large, long-term trial to evaluate the long-term safety of doxycycline in patients with rheumatoid arthritis. The researchers will conduct a large, multi-center, double-blind, placebo-controlled study of the drug in the next three years, when they conduct the first large, long-term study of doxycycline in patients with rheumatoid arthritis.
